COVID-19 Vaccine Registration for Students Age 12 and Older. Registration Open for Dose #1 on Friday, May 14

PARENTAL PERMISSION FORM
The COVID-19 vaccine has now been approved for children age 12 and older. Maumee City Schools is working with the Toledo Lucas County Health Department (TLCHD) to offer a vaccination option for Maumee students age 12 and older (as of May 14, 2021), as well as other qualified members of their family and members of our community. The school district recognizes that receiving or not receiving the COVID vaccination is a personal decision. We will continue to work in cooperation with TLCHD to make vaccinations convenient for those who decide to have their children vaccinated and at the same time support those who decide not to have their children vaccinated. 

A parent or guardian does not need to attend with Maumee High School students who are vaccinated during the school day but they MUST sign the parental permission slip in advance if their child is under 18. MHS students must bring the completed permission form and a photo ID with them to their vaccination appointment.  Gateway & Wayne Trail students may attend this vaccination clinic but must be accompanied by a parent/legal guardian. 

This is a two-vaccination event utilizing the Pfizer vaccine. Participants need to receive the vaccine on both dates at Maumee High School: 

Friday, May 14, 2021 (Dose #1) Appointments are 1 – 6 p.m. (parent will self-schedule and it may be in a time slot during the school day.) MHS students will return to class. GMS & WT parents may sign their students out early from the school office to take them to the vaccination clinic. 

Friday, June 4, 2021 (Dose #2) Appointments are 1 – 6 p.m. (parent will self-schedule and it may be in a time slot during the school day.) MHS students will return to class. GMS & WT parents may sign their students out early from the school office to take them to the vaccination clinic.  

Appointment times for dose #1 will be carried over for dose #2 (For example, if an individual receives dose #1 on 5/14/21 at 4:30 p.m., his/her appointment for dose #2 will automatically be scheduled for 6/4/21 at 4:30 p.m.) 

The site for both vaccination appointments is Maumee High School (Gym Annex) – 1147 Saco Street. Please park in the lot adjacent to the ANNEX entrance at Door #28. There will be signage throughout the MHS campus to clearly direct community members to the vaccine clinic. 

Please follow the steps below as soon as possible to register. Click on the following link: https://gettheshot.coronavirus.ohio.gov/ and:
1.  Select Eligibility and Book My Appointment. 
2.  Complete the COVID-19 Vaccination Questionnaire – YOU WILL NEED TO ENTER THE ACCESS CODE - BSYB9CM3IU – the form says “optional” but you must enter the code to participate in the Maumee High School vaccination clinic
3.  Enter Maumee High School in the location search box
4.  Click See Availability
5.  Make sure the correct date is entered and select your preferred time
6.  Click Continue
7.  C
omplete all the information and continue through the rest of the registration form.
